Abstract

|
Distal (type I , classical) renal tubular acidosis is characterized by metabolic acidosis secondary to an inability of the cells of the distal nephron to produce a sufficient hydrogen ion gradient between blood and urine regardless of the degree of systemic acidosis. Authors recently had a chance to examine a 25 years old female patient who was admitted to the Seoul National University Hospital because of periodic paralysis and vomiting. The patient presented with systemic metabolic acidoids without hyperchloremia, inability of urinary concentration and acidification, potassium depleting nephropathy. The patient was subsequently treated by a mixture of potassium and sodium bicarbonate with good response.
